2. Secure Locational Clearance (4/F City Planning Office)

3. Secure Fire Clearance (2/F BPLO Lounge)

4. Secure Building Clearance (2/F Building Official)

5. Proceed to Treasurer’s Office for approval of Gross Sales by OIC-Treasurer (G/F City Treasurer’s Office)

6. Proceed to EDP for Billing & Assessment (2/F BPLO Lounge)

7. Proceed to Treasurer’s Office for approval of billing statement by OIC-Treasurer (G/F City Treasurer’s Office)

8. Proceed to cashier for payment (G/F Taxpayer’s Lounge)

9. Proceed to City Health Dept. for the issuance of Sanitary Permit (G/F City Health Dept.)

10. Proceed to BPLO for the issuance of Claim Slip (2/F BPLO Lounge)

11. Approval and Releasing of Mayor’s / Business Permit by OIC-BPLO (2/F BPLO)

* In case SEC or DTI approved date of application is NOT CURRENT with date of application for Local Business Permit.

* In case applicant has existing business permit in other cities/municipalities. (for New Business only
)

1. Business Permit in other City/Municipality (photocopy)

2. Registration Cert. from BIR –Pque. Branch (photocopy w/original)

3. Retirement from other city (if transferred) (photocopy)

4. BIR stamped form 1701/1702 of prior years until current year (photocopy w/ original)

5. Current Cedula from other City/Municipality (photocopy)

* In case submitted Financial Statement on gross sales is CONSOLIDATED. (for Renewal of Business only)

1. Gross Sales summary of Principal/Branches signed by authorized signatory (original copy)

2. Statement of Account (S.O.A) with gross sales declaration and its corresponding official receipt. (photocopy)
